The Self-Directed Care Program is for adults with behavioral health issues. The program offers one-on-one peer support and advocacy services.
Self-Directed Care (SDC) is a unique person-centered program; the only one in Maryland, in which staff work with individuals to help them identify health, employment and educational goals and to provide the tools they need to achieve these goals. This is a 1:1 program in which a peer advocate works with an individual to achieve life goals for a better life in as many domains of their lives as possible. One difficulty is that SDC's participants are virtually all living below the FPL(Federal Poverty Level) and face frequent challenges in maintaining stable life situations because of this. SDC is open 8:30-3:30 M-F.
A mental health provider referral is necessary.
SELF-DIRECTED CARE PROGRAM
PERSON CENTERED PLANNING
The peer support staff can assist those individuals who are referred by a Mental Health provider by;
- Developing a personal recovery plan using WRAP (Wellness Recovery Action Plan)
- Defining individual goals and personal objectives including securing housing, education, or employment
- Providing transportation as available to attend entitlement appointments
- Locating appropriate services and any necessary supports to gain an increased level of independence
- Open Monday - Friday from 8:30 AM –2:30 PM by appointment.
SELF-DIRECTED CARE SUPPORT
Support and guidance provided to individuals in a self-directed process. The staff will build on strengths, identify challenges and help to develop a plan of action. The staff supports all who wish to become better advocates for themselves. The staff are able to assist individuals with the ways to cope with any challenges and issues.
Staff are peer support individuals who understand through "lived experiences."
SELF-DIRECTED CARE SERVICES
Support - Groups or one-on-one sessions to have participants and staff help each other
in alternative coping strategies through lived experiences.
Assistance - link up with other programs in the community
Information and Education - Community partners and peers sharing their knowledge in
an open, safe environment on a daily basis or as needed

The program is staffed by peers who are there to assist with issues of:
Mental Health, Behavioral Health, Substance Use Disorders, Independence/Housing
The program provides a network and programming to assist adults on the road to recovery and independence and works to promote wellness in our vision of peer support and advocacy. The SDC program is on a referral basis from mental health providers.
